
The telephone book problem
A TMF can be 96% complete and still fail to tell the story of a trial.
Completeness scores only measure whether expected documents are present. They cannot reveal missing context, inconsistencies, or the records that were never on the list but matter in an inspection.
The real question is not “Is everything filed?” but “Can someone reconstruct what happened, why, and by whom?” A file can be complete yet fragmented—like a telephone book full of numbers but no narrative.
The goal is coherence, not just collection: records that come together to form a clear, inspectable story of the trial.
